Job summary

Mission Healthcare seeks a Speech-Language Pathologist to provide home health services in Concord/Fairfield, CA on a PRN basis. You'll evaluate and treat patients with speech, language, voice, and swallowing disorders, coordinating with physicians and the clinical team.

Requires a Master’s in Speech-Language Pathology, CCC-SLP, state licensure, and BLS/CPR certification. This role offers flexible scheduling within a compassionate, inclusive care environment.

Qualifications

• Master’s degree in Speech-Language Pathology from an accredited program.
• CCC-SLP certification and state licensure to practice.
• BLS (CPR) certification required.
• Strong communication, documentation, and organizational skills.

Responsibilities

• Conduct thorough speech-language evaluations and assess the home environment to support therapy goals.
• Develop and implement personalized plans of care for patients with speech, language, voice, and swallowing disorders.
• Provide therapy using specialized techniques and equipment.
• Coordinate care with referring physicians and clinical team members.
• Document evaluations, treatment plans, and progress accurately.
• Supervise home health aides when related to the plan of care.
• Educate patients and families on therapy goals and progress.
• Maintain up-to-date knowledge of SLP practices.

Pay range (depending on experience): Starting at $70/hour!

Schedule/Shift: PRN as needed

Territory/Location: Concord and/or Fairfield!

Responsibilities

• Conduct thorough speech-language evaluations and assess the home environment to support therapy goals.
• Develop and implement personalized plans of care for patients with speech, language, voice, and swallowing disorders.
• Provide therapy using specialized techniques and equipment tailored to each patient’s needs.
• Recommend appropriate communication aids and devices when necessary.
• Coordinate care with referring physicians and other clinical team members.
• Participate in case conferences, training sessions, and interdisciplinary meetings to ensure cohesive care delivery.
• Accurately document evaluations, treatment plans, and progress in accordance with agency policies.
• Maintain up-to-date knowledge and skills in speech-language pathology practices.
• Identify additional needs of patients and their families and make appropriate referrals.
• Educate patients, caregivers, and family members on therapy programs and goals.
• Supervise home health aides when services are directly related to the speech-language plan of care.
• Support overall patient care delivery through additional duties as assigned.

Qualifications

• Master’s degree in Speech-Language Pathology from an accredited program.
• Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from ASHA.
• State license to practice as a Speech-Language Pathologist.
• At least one year of clinical experience in speech-language pathology (home health experience preferred).
• Current BLS (CPR) certification – hands‑on training required.
• Excellent communication, documentation, and organizational skills.
