Job summary

The Encore at Boca Raton Rehabilitation and Nursing Center is seeking a Speech Language Pathologist to join its rehabilitation team. The SLP will assess patients requiring rehabilitation, develop and administer a tailored treatment plan, and collaborate with the care team to achieve optimal patient outcomes.

Qualified candidates must have a Master’s in Speech-Language Pathology, a Florida license, and experience in long-term or geriatric care.

Qualifications

• Graduated from an accredited Speech Therapist college or university.
• Must have a valid professional license and registration for state of FL.
• Experience with long-term care or geriatric care is preferred.
• Well versed with documentation writing.
• Evaluations, weekly progress notes, recertification and discharge documentation.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer a treatment program with goals aligned to patient capacity.
• Document patient condition and educate patients and families in therapy approaches.
• Coordinate with other disciplines to provide patient-centered care.

The Encore at Boca Raton Rehabilitation and Nursing Center is looking for a talented and hard-working Speech and Language Therapist to join their ever-growing team. Speech Language Pathologist (SLP) Responsibilities Include:

The Speech Language Pathologist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Speech Language Pathologist will establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician. As well, the Speech Language Pathologist must set realistic and achievable goals for their patients, document and record the patients condition and educate patients and families in an appropriate therapy method.

Speech Language Pathologist (SLP) Qualifications/Requirements:

• Must have graduated from an accredited Speech Therapist college or university.

• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
• Experience with Case Mix Index and PPS scheduling is preferred however not limited to.
