Job summary

The Encore at Boca Raton Rehabilitation and Nursing Center is seeking a skilled Speech Language Pathologist to join our rehabilitation team. You will assess referred patients, develop and implement therapy plans, and document progress consistent with physician directions and regulatory requirements.

Ideal candidates hold FL licensure, have experience in long-term or geriatric care, and demonstrate strong documentation and collaboration with the care team to deliver exceptional patient-centered

Qualifications

• Must be licensed as a Speech-Language Pathologist in Florida and maintain licensure.
• Experience in long-term care or geriatric settings is preferred.
• Excellent documentation-writing skills and ability to maintain accurate records.
• Strong collaboration with multidisciplinary teams to deliver patient-centered care.
• Ability to provide direct patient care, assess conditions and create treatment plans under physician direction.
• Punctual, professional, and compliant with confidentiality, HIPAA and residents' rights.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer treatment programs with goals aligned to patient capacity.
• Set realistic goals, document patient conditions, and educate patients and families.
• Record progress notes, recertifications and discharge documentation.
• Collaborate with all disciplines to ensure best patient-centered outcomes.
• Perform duties as assigned by Director of Rehabilitation.
• Adhere to confidentiality and regulatory requirements.

Speech Language Pathologist (SLP) Responsibilities Include:

The Speech Language Pathologist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Speech Language Pathologist will establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician. As well, the Speech Language Pathologist must set realistic and achievable goals for their patients, document and record the patients condition and educate patients and families in an appropriate therapy method.

Speech Language Pathologist (SLP) Qualifications/Requirements:

• Must have graduated from an accredited Speech Therapist college or university.
• Must have a valid professional license and registration for state of FL
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
• Experience with Case Mix Index and PPS scheduling is preferred however not limited to.
