Job summary

FOX Rehabilitation in Waterloo, IA is seeking a licensed Speech Language Pathologist to deliver one-on-one care in private residences and senior living communities, within our Geriatric House Calls model.

You will design individualized plans, perform evaluations, and provide therapy with flexibility to create your own schedule, including telehealth when appropriate, to support aging in place and meaningful patient outcomes.

Qualifications

• Licensed Speech-Language Pathologist in the state(s) of practice or eligible.
• Degree from an accredited Speech-Language Pathology program.
• EMR proficiency and CPR certification.

Responsibilities

• Provide one-on-one speech-language pathology services to older adults in homes or senior living settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy to support wellness, longevity, mobility, and independence.
• Document in the EMR per regulatory standards and collaborate with an interdisciplinary team.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth / virtual care when appropriate.

Responsibilities

• Provide one-on-oneSpeech Language Pathologyto older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)

• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment

• Document effectively within FOX’s EMR systemin accordance withregulatory standards

• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists

• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place

• Provide telehealth / virtual care whenappropriate toextend access and flexibility

Qualifications

• ValidSpeech Language Pathologylicense in the state(s) of practice, or eligibility to obtain

• Degree from an accreditedSpeech Language Pathologyprogram

Physical Requirements

FOX clinicians must be able to meet the physical demands of in-home and community-based care, including:

• Exerting medium physical effort, including lifting 25-50lbsoccasionally, 10-25lbsfrequently, and up to 10lbsconstantly

• Performing CPR

• Providing moderate to maximumassistancewith patient transfers and bed mobility

• Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities

• Driving to patient homes and senior living communities as part of the Geriatric House Calls model

• Wearing PPE whenrequired
