Job summary

Jackson Therapy Partners is seeking a Traveling Speech-Language Pathologist in Santa Rosa, CA. You will deliver SLP services in home health settings, evaluating and treating communication and swallowing disorders while coordinating with caregivers and medical teams.

The role requires a Master’s in Speech-Language Pathology, CFY/CCC, and an active CA license. BLS may be required. Travel and housing support included; 20 hours per week over 2–3 days.

Qualifications

• Master's degree in Speech-Language Pathology is required.
• CFY and CCC from ASHA must be completed.
• Active CA license required to start.
• BLS certification may be required by employer.
• 1 year of recent skilled home health experience within the last 3 years.
• Shift hours are 8a-5p.
• Traveling SLP must have own transportation.
• Occasional travel radius of 50+ miles.

Responsibilities

• Provide speech-language pathology services to patients in home health.
• Assess and treat communication and swallowing disorders.
• Collaborate with families and care teams to implement treatment plans.
• Travel to multiple patient sites as required, up to ~6 patients per day.
