Qualifications

• Must hold a Master’s Degree in Speech-Language Pathology and an active license in the state of practice.
• Must possess basic computer skills and be able to navigate service delivery software and electronic health record systems.

• Return patients to optimal functional status in a SNF setting and facilitate achievement of therapy goals.
• Instruct patients in maintaining function post-discharge through education and independent exercise.
• Collaborate with physicians and therapy team; document progress in EHR.
