Job summary

Continuum Rehab Group is seeking a Speech Language Pathologist CF or CCC to join our in-house therapy team serving a Skilled Nursing/Long Term Care Facility. The role focuses on evaluation, treatment, and ongoing communication with residents to improve intelligibility, swallowing safety, and overall quality of life.

Responsibilities

• Direct all aspects of quality patient care, including evaluation, treatment, planning, goal setting, family education and documentation per guidelines.
• Provide professional treatment for speech, language, hearing, oral motor, swallowing and cognitive disorders.
• Plan activities and provide therapeutic services for speech and language disorders.
• Perform patient evaluations and develop treatment plans with short- and long-term goals per physician orders.
• Carry out patient care plans and progression within the established care plan.
• Develop effective treatment plans and obtain physician approval for services.
• Collaborate with an occupational therapist, physical therapist, physicians and other health-care team members.
• Document evaluations, progress notes and discharge summaries promptly.
• Maintain compliant documentation and Plan of Care updates on an ongoing basis.
• Ensure delivery of quality rehab services in the SNF.
• Demonstrate courtesy, compassion and respect to patients, caregivers and colleagues.
• Adhere to the company’s Code of Conduct and Compliance Program.
