Job summary

Evolve Therapy Services, LLC is hiring a Speech Language Pathologist for an in-house Skilled Nursing Facility in Dunn Loring, VA. The role is offered as Full Time, Part Time, or PRN to support resident needs and maintain high clinical standards.

Responsibilities include evaluation, treatment planning, family and staff instruction, and CMS-compliant documentation in NetHealth EMR. Certification and licensure are required, with opportunities for growth within the organization.

Qualifications

• Master's degree in Speech Language Pathology is required.
• CCC-SLP certification is required for practice.
• Active state licensure in practice state(s) is mandatory.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide quality speech therapy services and initiate referrals as needed.
• Evaluate and collect data for treatment planning and implementation.
• Develop treatment plans with target lists and goals.
• Instruct patient, family, and staff on speech-language procedures for carryover.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols with site coordination.
• Participate in resident needs screening with clinical excellence.

Job Position:

Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type:

Full Time, Part Time, or PRN

Job Purpose:

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
