Job summary

Evolve Therapy Services, LLC seeks a Speech-Language Pathologist to support an in-house Skilled Nursing Facility in Ahoskie, NC. The role covers full-time, part-time, or PRN schedules with focus on quality resident care and compliance standards.

The SLP will evaluate residents, develop plans, educate families, and ensure CMS-compliant documentation using NetHealth EMR. License in-state and CCC are required.

Qualifications

• Master's degree in Speech Language Pathology is required.
• CCC (Certificate of Clinical Competency) is required.
• Active SLP licensure in relevant states is required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals as needed.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with target lists and goals (short and long term).
• Instruct patients, families, and staff in speech-language procedures for carryover of remediation.
• Complete CMS-mandated documentation in NetHealth EMR systems.

Job Position

Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type

Full Time, Part Time, or PRN

Job Purpose

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
