Job summary

Evolve Therapy Services is seeking a qualified Speech Language Pathologist to join our in-house team at a Skilled Nursing Facility in Natick, MA. The role involves evaluating, planning, and directing SLP services to ensure high-quality resident care and compliance with standards.

Responsibilities include delivering therapy, documenting CMS-compliant records in NetHealth EMR, and collaborating with staff to support daily activities and patient goals.

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency.
• Active SLP license in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide quality speech therapy services and initiate referrals according to standards and procedures.
• Evaluate and obtain data for treatment planning and implementation.
• Develop a treatment plan with short-term and long-term goals.
• Instruct patient, family, and staff in speech-language procedures for carryover.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Screen residents to ensure needs are met timely with clinical excellence.

Job Position:

Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type:

Full Time, Part Time, or PRN

Job Purpose:

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
