Job summary

Evolve Therapy Services, LLC is seeking a Speech Language Pathologist to join our in-house Skilled Nursing Facility in Butler, Alabama on a PRN basis. The role involves planning, developing, and directing SLP services, evaluating data for treatment planning, creating short- and long-term goals, educating patients and families, and ensuring CMS-compliant documentation in NetHealth EMR.

Candidates must have a Master's in Speech Language Pathology, CCC, and an active SLP license in the state(s) of

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency.
• Active SLP license in state(s) of practice.

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice.
• Evaluate and obtain data necessary for treatment planning and implementation.
• Develop a treatment plan that includes a target list, short-term, and long-term goals.
• Instruct patients, families, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities.
• Complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems.
• Oversee and implement proper protocols for FEES/MBS with coordination of site programs.
• Participate in screening protocols for establishing needs of residents timely and with clinical excellence.

Job Type:

Job Purpose:

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
