Evolve Therapy Services, LLC is seeking a Speech Language Pathologist to join our in-house Skilled Nursing Facility partner in Franklin, MN. The role focuses on planning and delivering comprehensive speech-language pathology services to residents, with emphasis on quality care and regulatory compliance.

The ideal candidate holds a Master's in Speech Language Pathology, CCC, and an active SLP license, plus ongoing continuing education.

Qualifications

• Master's degree in Speech Language Pathology.
• CCC - Certificate of Clinical Competency.
• Active SLP license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate.
• Evaluate and obtain data necessary for treatment planning and implementation.
• Develop a treatment plan that includes a target list, short-term, and long-term goals.
• Instruct patient, family, and staff to carry over remediation and goals in daily activities.
• Complete CMS-compliant documentation from CMS expectations in NetHealth EMR systems.
• Oversight and implementation of FEES/MBS protocols with site coordination.
• Participate in screening protocols to meet resident needs with clinical excellence.

Job Position:

Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type:

Full Time (offering $5,000 sign on bonus and tuition reimbursement!), Part Time, or PRN

Job Purpose:

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
