AmeriStaff Nursing Services is seeking compassionate Speech Language Pathologists (SLP) to deliver high-quality, patient-centered care directly to patients in their homes in Troy, MI. The role emphasizes autonomy, a flexible schedule, and direct impact on patient outcomes in a home-health setting.

The position offers a per-visit compensation of $85 for each completed 1-hour session, with a commitment to high-quality care and collaboration with the interdisciplinary team.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist (PT) license in the State of Michigan.

Responsibilities

• Evaluate patient function and develop individualized treatment plans in collaboration with the physician.
• Perform diagnostic and functional assessments (muscle, nerve, joint, and mobility).
• Provide therapeutic interventions to relieve pain and improve mobility and independence.
• Educate patients and families on exercises, treatment plans, and use of assistive devices (canes, walkers, wheelchairs, etc.).
• Communicate regularly with the Nurse/Case Manager and interdisciplinary team.
• Document patient progress, treatment responses, and changes in condition in a timely manner.
• Participate in development and updates of the Plan of Care.
• Coordinate outpatient services when needed.

Education

Graduate of an accredited Physical Therapy program

Current Physical Therapist (PT) license in the State of Michigan

In this role, you will evaluate patients to identify speech, language, voice, and swallowing disorders through clinical assessments, standardized tests, and patient interviews —all within the comfort of their homes.

Key Responsibilities:

Evaluate patient function and develop individualized treatment plans in collaboration with the physician

Perform diagnostic and functional assessments (muscle, nerve, joint, and mobility)

Provide therapeutic interventions to relieve pain and improve mobility and independence

Educate patients and families on exercises, treatment plans, and use of assistive devices (canes, walkers, wheelchairs, etc.)

Communicate regularly with the Nurse/Case Manager and interdisciplinary team

Document patient progress, treatment responses, and changes in condition in a timely manner

Requirements:

Ability to meet physical demands including lifting, transferring, and assisting patients
