Job summary

Gallatin Center for Rehabilitation and Healing in Lakewood, Tennessee, is seeking a Speech Language Pathologist (SLP) to join our rehabilitative team as a full-time or PRN clinician.

You will assess referred patients, develop and administer treatment plans, document progress, and educate patients and families under physician direction. TN licensure required; long-term/geriatric care experience preferred.

Qualifications

  • Graduated from an accredited Speech Language Pathology program.
  • Valid Tennessee state license and registration.
  • Experience in long-term care or geriatric care preferred.
  • Strong documentation/writing skills.
  • Ability to collaborate with all disciplines for patient-centered care.
  • Punctuality and professionalism in care delivery.
  • HIPAA compliance and Residents' rights awareness.

Responsibilities

  • Assess referred patients requiring rehabilitation and provide direct patient care.
  • Develop and administer a treatment program with specific goals according to patient capacity and tolerance under physician direction.
  • Document and record patient condition; educate patients and families in appropriate therapy methods.
  • Ensure confidentiality and compliance with HIPAA and residents' rights.
  • Perform duties as assigned by the Director of Rehabilitation.

Gallatin Center for Rehabilitation and Healing is seeking a Speech Language Pathologist (SLP) to join our team.

Speech Language Pathologist (SLP) Responsibilities Include:

The Speech Language Pathologist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Speech Language Pathologist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. As well, the Speech Language Pathologist must set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families in an appropriate therapy method.

Speech Language Pathologist (SLP) Qualifications/Requirements:
  • Must have graduated from an accredited Speech Therapist college or university.
  • Must have a valid professional license and registration for state of TN
  • Experience with long-term care and/or geriatric care is preferred but not limited to.
  • Well versed with documentation writing
  • Evaluations, weekly progress notes, recertification and discharge documentations.
  • Collaborates with all disciplines to provide best patient centered treatment and care.
  • Exceptional patient service skills and ability to demonstrate clinical excellence
  • Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
  • Perform all other duties assigned by Director of Rehabilitation
  • Compliance with confidentiality, HIPPA and Federal Residents Rights.
