Centra Healthcare Solutions, Inc. is seeking a Speech Language Pathologist (SLP-CCC) for a contract assignment in a Skilled Nursing Facility / Long-Term Care setting in Houma, LA.

The role requires current state licensure or active pursuit and CPR certification. Expect a contract position with a days schedule and responsibility for delivering skilled SLP services per physician orders, assessing patient needs, and collaborating with an interdisciplinary team to meet communication and swallowing

Qualifications

• Must have graduated from an accredited school.
• Current state license as an SLP or actively pursuing licensure.
• Current CPR certification.
• SLP-CCC credential required.
• Must work within an interdisciplinary team to develop a care plan.
• Provide skilled SLP services per physician orders.
• Assess patient needs and modify care to meet goals.
• Set realistic goals for communication and swallowing.

Job Description

A great SNF / LTC is looking for an experienced Speech Language Pathologist / SLP to join their growing team for a contract assignment!

• This is a contract position.
• Days schedule.
• This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.

Requirements include

• Must have graduated from an accredited school.
• Current state license as a Speech Language Pathologist / SLP or proactively in the process of the application process for current state licensure as a Speech Language Pathologist / SLP.
• Current CPR certification.
• Work within an interdisciplinary team to develop a plan of care for each patient.
• Provide skilled speech therapy services / interventions in accordance with physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of speech therapy interventions.
• Set realistic and achievable goals for patients to develop, or recover, reliable communication and swallowing skills so that the patient can fulfill their educational, vocational and social roles
• Candidate must be an SLP-CCC.
