Job summary

Centra Healthcare Solutions is seeking an experienced Speech Language Pathologist (SLP-CCC) for a SNF/LTC setting in Sunnyvale, CA. This is a direct hire role with a standard schedule and requires CCCs and active state licensure or licensure in process.

CPR certification is also required. The SLP will work within an interdisciplinary team to provide skilled therapy, follow physician orders, and set achievable goals to improve communication and swallowing for patients.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Speech Language Pathologist / SLP or be in the process of licensure.
• Must have CCCs.
• Current CPR certification.
• Work within an interdisciplinary team to develop a plan of care for each patient.
• Provide skilled speech therapy services / interventions in accordance with physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of speech therapy interventions.
• Set realistic and achievable goals for patients to develop, or recover, reliable communication and swallowing skills.

A great SNF / LTC is looking for an experienced Speech Language Pathologist / SLP to join their growing team! Candidate MUST be an SLP-CCC!

• This is a direct hire / 'perm' position.
• Days schedule.
• This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.

Requirements include:

• Must have graduated from an accredited school.
• Current state license as a Speech Language Pathologist / SLP or proactively in the process of the application process for current state licensure as a Speech Language Pathologist / SLP.
• Must have CCCs.
