Job summary

Heights Care and Rehab is seeking a compassionate Speech-Language Pathologist (SLP) to assess residents with communication, cognitive, speech, language, voice, and swallowing disorders. You will create individualized treatment plans to maximize outcomes and provide evidence-based therapy with a focus on safety and quality of life.

Qualifications

  • Current Colorado Speech-Language Pathologist license required.
  • Master's Degree in Speech-Language Pathology.
  • CCC-SLP certification preferred; CF candidates considered.
  • Strong clinical, communication, and interpersonal abilities.

Responsibilities

  • Evaluate residents with communication, cognitive, speech, language, voice, and swallowing disorders.
  • Develop individualized treatment plans designed to maximize resident outcomes.
  • Provide evidence-based speech therapy interventions.
  • Assess and manage dysphagia, swallowing safety, and dietary recommendations.
  • Collaborate with nursing, dietary, therapy teams, physicians, and families to ensure coordinated care.
  • Participate in care planning and discharge planning meetings.
  • Educate residents, families, and caregivers on treatment strategies and safety recommendations.
  • Maintain accurate and timely documentation.
  • Help residents achieve the highest possible level of independence and quality of life.

What You'll Do

What We're Looking For
  • Current Speech-Language Pathologist license in the State of Colorado
  • Master's Degree in Speech-Language Pathology
  • Certificate of Clinical Competence (CCC-SLP) preferred
  • Strong clinical, communication, and interpersonal skills
  • Passion for improving the lives of older adults
  • Skilled Nursing Facility experience preferred, but not required
  • Clinical Fellowship candidates may be considered
