Glengariff Rehabilitation and Healthcare Center is looking for a Speech Language Pathologist (SLP) to assess and treat patients in a sub-acute and long-term care setting. The SLP will establish and administer individualized treatment plans, document patient progress, and collaborate with an interdisciplinary team to improve patient functioning.

Requirements include graduation from an accredited SLP program and a valid NY license.

Qualifications

  • Graduate from an accredited Speech-Language Pathology program.
  • Valid NY state license and registration.
  • Experience in long-term/geriatric care preferred.
  • Proficient in clinical documentation.
  • Document evaluations, progress notes, re-certifications and discharge docs.
  • Collaborate with interdisciplinary team for patient-centered care.
  • Be punctual with scheduled hours and professional.
  • Comply with HIPAA and Residents Rights.

Speech Language Pathologist (SLP)

Salary: Up to $45 an hour

Speech Language Pathologist (SLP) Responsibilities Include:

The Speech Language Pathologist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Speech Language Pathologist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. As well, the Speech Language Pathologist must set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families in an appropriate therapy method. Work along side and as a team member of an interdisciplinary team formulating facility wide programming, policy, audits, and processes to improve the life and function of all patients.

Speech Language Pathologist (SLP) Qualifications/Requirements:
  • Must have graduated from an accredited Speech Therapist college or university.
  • Must have a valid professional license and registration for state of NY
  • Experience with long-term care and/or geriatric care is preferred but not limited to.
  • Well versed with documentation writing
  • Evaluations, weekly progress notes, re-certification and discharge documentations.
  • Collaborates with all disciplines to provide best patient centered treatment and care.
  • Exceptional patient service skills and ability to demonstrate clinical excellence
  • Report to work on time; adhere to scheduled hours and demonstrate professionalism at all times.
  • Perform all other duties assigned by Director of Rehabilitation
  • Compliance with confidentiality, HIPPA and Federal Residents Rights.
  • Experience with Case Mix Index and PPS scheduling is preferred however not limited to.

Salary: Up to $45 an hour
