Job summary

Glengariff Healthcare Center seeks a Speech Language Pathologist (SLP) to assess referred patients needing rehabilitation services and deliver direct patient care. The role includes creating a tailored treatment plan, documenting progress, and educating families under physician direction.

The SLP will collaborate with an interdisciplinary team to ensure patient-centered care in a sub-acute/long-term care setting. NY licensure is required, prior long-term care experience is preferred.

Qualifications

  • Must have graduated from an accredited Speech Therapist college or university.
  • Must have a valid NY license and registration.
  • Experience with long-term care and/or geriatric care is preferred.
  • Well versed with documentation writing.
  • Collaborates with all disciplines to provide patient-centered treatment and care.

Speech Language Pathologist (SLP)

Salary: Up to $45 an hour

Speech Language Pathologist (SLP) Responsibilities Include:

The Speech Language Pathologist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Speech Language Pathologist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. As well, the Speech Language Pathologist must set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families in an appropriate therapy method. Work along side and as a team member of an interdisciplinary team formulating facility wide programming, policy, audits, and processes to improve the life and function of all patients.

Speech Language Pathologist (SLP) Qualifications/Requirements:

  • Must have graduated from an accredited Speech Therapist college or university.
  • Must have a valid professional license and registration for state of NY
  • Experience with long-term care and/or geriatric care is preferred but not limited to.
  • Well versed with documentation writing
  • Evaluations, weekly progress notes, re-certification and discharge documentations.
  • Collaborates with all disciplines to provide best patient centered treatment and care.
  • Exceptional patient service skills and ability to demonstrate clinical excellence
  • Report to work on time; adhere to scheduled hours and demonstrate professionalism at all times.
  • Perform all other duties assigned by Director of Rehabilitation
  • Compliance with confidentiality, HIPPA and Federal Residents Rights.
  • Experience with Case Mix Index and PPS scheduling is preferred however not limited to.

Salary: Up to $45 an hour
