Job summary

Homeside Rehab is seeking a compassionate Speech-Language Pathologist (SLP) to join our home health team in Coral Gables, FL. You will provide individualized, evidence-based therapy to patients in their homes with flexible scheduling and a supportive, collaborative environment.

You will evaluate communication disorders, develop tailored treatment plans, and deliver therapy for speech, language, fluency, and voice.

Qualifications

• Active Florida State Speech-Language Pathologist License required.
• Minimum 1 year of home health, outpatient, or SNF experience preferred.
• Strong documentation and communication skills.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide evidence-based speech, language, fluency, articulation, and voice therapy.
• Maintain accurate documentation of care in the EHR.
• Collaborate with clinical team for coordinated care.
• Communicate with patients, caregivers, and office staff.
• Coordinate with therapists, nurses, and physicians as needed.

Job Summary

Homeside Rehab is seeking a compassionate and skilled Speech-Language Pathologist (SLP) to join our home health team in the CORAL GABLES, FL 33134. In this role, you will provide individualized, evidence-based speech therapy services to patients in their homes while enjoying schedule flexibility and a supportive team environment.

Requirements

• ActiveFlorida State Speech-Language Pathologist License — required
• Minimum 1 year of experience in Home Health, Outpatient, or Skilled Nursing Facility (SNF) settings preferred
• Basic computer literacy for documentation and communication
• Strong clinical and interpersonal communication skills
• Ability to work independently in a home health environment
• Reliable transportation for home visits preferred

Job Responsibilities

• Evaluate patients and develop individualized treatment plans based on patients' needs and goals
• Provide evidence-based treatment for speech, language, fluency, articulation, and voice disorders
• Maintain accurate and timely documentation of patient care
• Collaborate with the clinical team to ensure coordinated and effective treatment
• Communicate effectively with office staff, patients, and caregivers
• Collaborate with healthcare professionals, including speech therapy assistants, nurses, and physicians, to ensure comprehensive care
