The Grove at Valhalla Rehabilitation and Nursing Center is seeking a Speech Language Pathologist (SLP) to provide and direct Speech Therapy services, including assessment, treatment, and program development under physician orders.

The SLP will conduct screenings, develop treatment plans with measurable goals, coordinate care with patients and families, and participate in discharge planning while ensuring compliance with department procedures and infection control standards.

Qualifications

  • Master's Degree in Speech-Language Pathology.
  • Current state license or license eligible.

Responsibilities

  • Provide and direct Speech Therapy services to patients, including assessment and treatment, and development of programs.
  • Conduct screenings of patients to determine need for intervention.
  • Evaluate patients to obtain data for treatment planning and implementation; conduct specialized evaluations as indicated.
  • Interpret and communicate evaluation findings to patients and family and other team members.
  • Establish individual patient schedules in conjunction with the team.
  • Develop treatment plans with short and long term goals and methods to achieve them.
  • Coordinate treatment plan with patient, family and other team members.
  • Monitor patient response and modify treatment to attain goals per physician orders.
  • Recommend and obtain equipment to improve patient communication; instruct patient, family and staff in use of devices.
  • Participate in discharge planning.
  • Develop home or community programming to maintain and enhance patient performance.
  • Document results of assessments, treatment, follow-up, and termination of services.
  • Supervise students and volunteers per state regulations.
  • Maintain ongoing continuing education and professional development.
  • Record indirect and billable treatment times per department procedures and submit statistics.
  • Provide treatment per standards of practice and infection control guidelines.
  • Contribute to Quality Assurance programs as required.
  • Adhere to confidentiality standards.
  • Perform other duties as assigned.

Speech Language Pathologist (SLP)

The Grove at Valhalla Rehabilitation and Nursing Center is currently searching for a Speech Language Pathologist. The SLP will provide and direct Speech Therapy services to patients to include assessment and treatment, development and implementation of programs, and related professional in-services. Performs duties under the direction of physician’s orders.

Speech Language Pathologist (SLP) Responsibilities:
  • Provides Speech Therapy services to patients and initiates referrals when appropriate and in accordance with Speech Therapy Department procedures.

  • Conduct screenings of patients at regular intervals to determine need for intervention/treatment.

  • Evaluates patients to obtain data necessary for treatment planning and implementation. Conducts specialized evaluations as indicated.

  • Interprets and communicates evaluation findings to patients, family, and other team members.

  • Establishes individual patient schedules in conjunction with other team members.

  • Develops treatment plans by identifying a problem list, including short and long term goals and methods to achieve identified goals.

  • Coordinates treatment plan with patient, family, and other team members.

  • Monitors patient’s response to intervention and modifies treatment as indicated to attain goals in accordance with physician orders.

  • Recommends and obtains appropriate equipment to improve patient’s communication. Instructs patient, family and staff in use of augmentative communication devices.

  • Participates in discharge planning.

  • Develops appropriate home or community programming to maintain and enhance the performance of the patient in their own environment.

  • Documents results of patient’s assessment, treatment, follow-up, and termination of services.

  • Supervises students and volunteers in accordance with state regulations.

  • Assumes responsibility for ongoing continuing education and professional development.

  • Records indirect and billable treatment times according to Speech Therapy Department procedures and submits weekly/monthly statistics to regional office.

  • Provides treatment in accordance with established standards of practice, department procedures and productivity standards.

  • Complies with all Infection Control, Universal Precautions and OSHA standards for the health care professional.

  • Assists with and participates in Quality Assurance Programs as required.

  • Adheres to established confidentiality standards.

  • Performs other duties as assigned.

Speech Language Pathologist (SLP) Requirements
