HOW YOU'LL MAKE A DIFFERENCE:

Care and compassion are central to your role as a Speech Therapist (ST). You'll join an interdisciplinary team focused on delivering compassionate, quality care and achieving positive patient outcomes. You'll interact with patients' families and be part of their recovery journey.

HOW YOU'LL WORK:
You will provide home-based skilled speech therapy as directed by the physician, educate patients and families, and ensure patient safety.
MAJOR AREAS OF RESPONSIBILITY:
Plan of Care: Develop and adjust individualized ST plans, guide patients in therapeutic activities, and report progress to physicians and staff.
Patient Outcomes: Use activities and exercises to improve speech, swallowing, cognition, and related functions.
Minimize Patient Risk: Educate patients on preventative measures to reduce re-hospitalizations.
Family Education/Advocacy: Counsel patients and families on managing conditions and therapy processes.
Policies: Complete clinical documentation per agency and Medicare guidelines.
Rules and Regulations: Follow agency policies and communicate schedule changes.
Operations: Attend training and meetings.
HARD & SOFT SKILLS:
Compassionate communication and positive attitude.
Patience and observance of details.
REQUIREMENTS:
Current Speech Therapist license in the state of employment.
Minimum one-year experience as an ST, preferably in home care or acute settings.
Valid driver’s license and auto insurance.
Ability to meet physical demands.
