Job summary

Personal Touch Home Health Care Services Inc. in Oregon seeks a Speech Language Pathologist to provide direct therapy in a skilled nursing facility, assessing, planning, and implementing individualized treatment to improve communication and swallowing functions.

Qualifications

  • MS or MA from an accredited speech therapy program.
  • ASHA CCC certification or CFY eligibility.
  • Current state licensure in good standing.
  • CPR certification required.
  • Strong communication and interpersonal skills.
  • Proficiency with Microsoft Office and therapy software.
  • Ability to work independently and as part of a team.

Responsibilities

  • Assess, set, and modify functional treatment goals for patients.
  • Deliver therapy per physician referral and treatment plans.
  • Chart progress accurately and maintain therapy documentation.
  • Utilize therapy software effectively and responsibly.
  • Supervise and evaluate Speech Therapy Clinical Fellows (for CCC-certified SLPs).
  • Participate in community health projects and liaise with patients, families, and support staff.
  • Maintain confidentiality, professionalism, and adherence to policies and regulations.
  • Demonstrate excellent customer service and teamwork.

Role Overview: The Speech Language Pathologist (SLP) provides direct speech therapy rehabilitation care to patients in a skilled nursing facility setting. This includes assessing, developing, and implementing individualized treatment plans to help patients improve their communication and swallowing functions. The SLP collaborates with an interdisciplinary team to deliver patient-centered care, ensuring safety, compliance, and quality outcomes.
