HOW YOU'LL WORK:

You will provide home-based skilled speech therapy care, educate patients and families, and ensure patient safety.

MAJOR RESPONSIBILITIES:

• Plan of Care: Develop and adjust individualized plans; guide patients in therapeutic activities; report progress.
• Patient Outcomes: Use activities and exercises to improve speech-related functions.
• Minimize Patient Risk: Teach preventative measures to reduce re-hospitalizations.
• Family Educator/Advocate: Counsel patients and families on disease management throughout therapy.
• Policies: Complete documentation per agency and Medicare guidelines.
• Rules and Regulations: Follow policies and communicate schedule changes.
• Operations: Attend training and meetings.

HARD & SOFT SKILLS:
