Manchester Center for Rehabilitation and Healing in Tennessee is seeking a dedicated Speech Language Pathologist (SLP) to join our skilled nursing team. The successful candidate will evaluate and treat residents independently, focusing on functional communication and swallowing goals.

Candidates must have an accredited SLP program degree and Tennessee licensure; geriatric experience is preferred. Strong documentation skills, teamwork, and professionalism are essential to deliver patient-centered

Qualifications

  • Must have graduated from an accredited Speech-Language Pathology program.
  • Must hold valid TN state license and registration.
  • Experience in long-term care and/or geriatric care is preferred.
  • Excellent documentation and record-keeping skills.
  • Collaborates with all disciplines to provide patient-centered care.
  • Complies with confidentiality, HIPAA, and Residents' Rights.

Responsibilities

  • Assess referred patients needing rehabilitation and provide direct care.
  • Develop and administer treatment plans with goals aligned to patient capacity.
  • Document patient condition and progress, including weekly notes.
  • Educate patients and families on therapy methods.
  • Coordinate with physicians and other disciplines to ensure quality care.

Speech Language Pathologist (SLP) Full Time and PRN

Manchester Center for Rehabilitation and Healing is looking for a talented and motivated Speech Language Pathologist (SLP) to join our dynamic team. The position is within a Skilled Nursing facility. Candidate should possess passion for geriatric population, must contain a license in the state of practice, be able to evaluate and treat independently without direct supervision, and posses the desire to perform functional therapeutic activities to assist residents improve in their experience.

Responsibilities Include:

The Speech Language Pathologist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Speech Language Pathologist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. As well, the Speech Language Pathologist must set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:
  • Must have graduated from an accredited Speech Therapist college or university.
  • Must have a valid professional license and registration for state of TN
  • Experience with long-term care and/or geriatric care is preferred but not limited to.
  • Well versed with documentation writing
  • Evaluations, weekly progress notes, recertification and discharge documentations.
  • Collaborates with all disciplines to provide best patient centered treatment and care.
  • Exceptional patient service skills and ability to demonstrate clinical excellence
  • Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
  • Perform all other duties assigned by Director of Rehabilitation
  • Compliance with confidentiality, HIPPA and Federal Residents Rights.
