Dahlia Gardens Center for Nursing and Rehabilitation in Aberdeen, NC is seeking a compassionate Speech Language Pathologist (SLP) to join our rehabilitation team. You will evaluate, plan, and deliver therapy to improve speech, language, and swallowing with caregiver guidance for continued success.

The role emphasizes interdisciplinary collaboration with nurses, physicians, dietitians, and families, along with documentation, discharge planning, and ongoing quality improvement initiatives.

Qualifications

• Licensed as an SLP in North Carolina.
• CCC-SLP preferred; CFs encouraged.
• Experience in skilled nursing/rehab is a plus.
• Strong communication and problem-solving skills.

Responsibilities

• Assess speech, language, cognitive-linguistic, voice, and swallowing disorders, and plan therapy.
• Deliver evidence-based therapy, with caregiver guidance and home programming.
• Collaborate with nurses, physicians, dietitians, families; participate in discharge planning.
• Document notes and billing; ensure Medicare compliance.
• Lead in-services and support quality improvement.

What You'll Do:

• Evaluate & Plan: Assess communication, cognitive-linguistic, voice, and swallowing disorders and create customized, functional treatment plans focused on recovery and quality of life.
• Deliver Therapy That Matters: Use evidence-based approaches to help patients improve speech, language, and swallowing, and provide caregiver guidance and home programming for continued success.
• Collaborate & Communicate: Work hand-in-hand with nurses, physicians, dietitians, and families, and participate in discharge planning and interdisciplinary care meetings.
• Document & Comply: Keep accurate, timely clinical notes and billing records, and ensure compliance with Medicare and all regulatory guidelines.
• Lead & Educate: Offer in-services for staff on speech and dysphagia care, and engage in ongoing quality improvement and team initiatives.

What You Bring:

• Licensed Speech-Language Pathologist (SLP) in the state of NC
• CCC-SLP preferred, but Clinical Fellows (CFs) are encouraged to apply!

• Experience in skilled nursing/rehab settings is a plus, but not required
