Job summary

FOX Rehabilitation is growing in Sacramento, CA, and we’re seeking passionate, licensed Speech Language Pathologists to deliver exceptional one-on-one care to older adults where they live. Roles include in-home visits or within Senior Living Communities, with optional telehealth for flexible scheduling.

You will create and manage individualized treatment plans, perform evaluations, deliver evidence-based therapy, and document care in FOX’s EMR system while collaborating with an interdisciplinary

Qualifications

• Valid SLP license in CA or eligible to obtain
• CCC certification by ASHA must be current or in process
• Degree from an accredited SLP program
• Current CPR certification

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in homes or senior living communities
• Perform evaluations, develop plans of care, deliver evidence-based treatment
• Deliver therapy sessions to support wellness, mobility and independence
• Document in FOX’s EMR per regulatory standards
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff to promote safety and aging in place
• Provide telehealth/virtual care when appropriate to extend access

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
• Document effectively within FOX’s EMR system in accordance with regulatory standards
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
• Provide telehealth / virtual care when appropriate to extend access and flexibility

• Part-time Speech Language Pathologist – In-Home & Senior Living Care (salaried with benefits eligibility based on minimum hours requirement)
• PRN / FOX Flex Speech Language Pathologist – Home & Community Visits (pay per unit compensation)

Qualifications

• Valid Speech Language Pathology license in the state(s) of practice, or eligibility to obtain
• Degree from an accredited Speech Language Pathology program
• Basic computer and EMR proficiency
• Current CPR certification
• Current Certificate of Clinical Competence (ASHA CCC)

Physical Requirements

• Exerting medium physical effort, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly
• Performing CPR
• Providing moderate to maximum assistance with patient transfers and bed mobility
• Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities
• Driving to patient homes and senior living communities as part of the Geriatric House Calls™ model
• Wearing PPE when required

In California, the standard base pay range for a Full-Time role is $80,000 - $110,000 annually with an opportunity to earn more. Part-Time and FOX Flex (PRN) will be pro-rated.
