Job summary

Sweetwater Care is seeking a licensed Speech-Language Pathologist to assist patients with communication disorders and swallowing safety in a SNF setting. You will evaluate, diagnose, and design treatment plans to improve intelligibility and swallowing function.

You will collaborate with physicians, nurses, therapists and families, document progress, and ensure compliance with Medicare/Medicaid guidelines while delivering compassionate, patient-centered care.

Responsibilities

• Direct all aspects of patient care including evaluation, treatment planning and documentation.
• Provide professional treatment using speech, language, hearing, oral motor, swallowing and cognitive rehabilitation techniques.
• Plan activities and provide therapeutic services for speech and language disorders.
• Serve as patient advocate and participate in quality assurance and customer service activities.
• Develop treatment plans and obtain physician approval.
• Document evaluations, progress notes and discharge summaries.
