Job summary

Pikes Peak Post Acute in Oregon seeks a licensed Speech-Language Pathologist to evaluate patients within 24 hours of referral, develop and obtain approval for treatment plans, and treat patients per the physician’s plan. You will supervise CFY/Speech Clinical Fellows and assist nursing staff with training of Restorative Aides.

Qualifications

Job description

Responsibilities
  • Evaluate patients within 24 hours, or 1 business day of physician referral. If unable to complete evaluation in 24 hours, document the cause for the delay in the medical record.
  • Develop an effective treatment plan and obtain approval for services from the referring physician.
  • Treat patients in accordance with the physician’s treatment plan.
  • Supervise Speech Language Pathologist Assistants in direct patient care and patient‑related activities, following the state practice act.
  • Communicate with supervisors and other health‑team members regarding patient progress, problems, and plans.
  • Participate in Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences as needed.
  • Participate in the services training program for other staff in the facility.
  • Record evaluations, daily treatment notes, weekly progress notes, recertification documentation per the Speech and Language Pathologist in the Board State Practice Act and governmental and third‑party payer requirements.
  • Record treatment changes per policy and procedures.
  • Instruct patients’ families or nursing staff in maintenance programs and caregiver training in preparation for discharge from therapy services.
  • Participate in discharge planning.
  • Act as a clinical preceptor for affiliating CFY/RFP Speech Language Pathologists and student programs.
  • Comply with the Speech Language Pathology and Audiology Bureau State Practice Act and the facility’s policies and procedures.
  • Relate positively, effectively, and appropriately with patients/residents, families, staff, and professional colleagues.
Supervisory Requirements
  • Supervise CFY – Speech Clinical Fellows, if applicable, and assist nursing staff with the training of Restorative Aides.
Qualifications
  • Education: Bachelor’s degree in Speech Language Pathology Therapy; preference for graduates of Master’s or Doctorate programs.
  • Proficient in computer skills such as data entry into programs such as Casamba and Point Click Care.
  • Language skills: Ability to read technical procedures, policy and procedure manuals, and to present information effectively and respond to questions from managers, coworkers, and families.
  • Mathematical skills: Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
  • Reasoning ability: Ability to solve practical problems and interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
Certificates, Licenses, Registrations
  • Licensed as a Speech Therapist in the state.
  • Employee must meet continuing education requirements per state practices.
  • Maintain license in good standing at all times with the state board.
