Job description

Overview

Concept Rehab is a therapist‑owned and operated company with over 40 years of experience. The Speech/Language Pathologist will provide competent, courteous, and respectful evaluation, treatment, and supervision of patients for communication, cognitive, problem‑solving, and swallowing difficulties. The role reports to the Therapy Program Manager and ultimately the Director of Operations.

Responsibilities

• Evaluate patients within 24 to 48 hours of physician orders.
• Develop and implement effective treatment plans.
• Deliver therapies according to the plan of care.
• Maintain professional documentation in the medical record.
• Communicate with physicians, clinical managers, other healthcare team members, patients, families, and facility staff regarding progress and plans.
• Participate in care conference meetings and interdisciplinary team meetings.
• Conduct in‑service training programs for facility staff.
• Provide objective input for discharge summaries and complete daily billing of services per policy and procedure.
• Instruct patients’ families or nursing staff in maintenance programs.
• Participate in discharge planning.
• Achieve or exceed the minimum productivity standard established by CRI.
• Recommend equipment that facilitates improved clinical service delivery and report equipment issues to appropriate personnel.
• Act as a clinical supervisor for affiliated Speech/Language Pathology graduate clinicians.
• Comply with state‑specific speech pathology licensure standards.
• Maintain an organized working environment and consistently portray the company mission, vision, and values.
• Execute special projects and other duties as assigned.

Qualifications

• Certified by the American Speech‑Language‑Hearing Association (ASHA).
• Licensed to practice Speech/Language Pathology in the state(s) where services are provided.
