Job summary

Elara Caring is seeking a Speech-Language Pathologist to evaluate patients, develop and implement plans of care, and provide therapy in home settings under OASIS and PPS guidelines.

The role requires travel within the branch area, a Master’s in Language Pathology, and an active SLP license. You will collaborate with medical teams to ensure safe, effective, ongoing patient care and discharge planning.

Qualifications

  • Master’s degree in Language Pathology from an accredited program.
  • Current, unrestricted SLP license in the state of work.
  • Minimum one year of experience as an SLP in a clinical care setting.
  • Willingness to travel within branch/office coverage area (50%).
  • Must have a dependable vehicle, valid driver’s license, and current auto insurance.

Responsibilities

  • Evaluate Speech-Language pathology patients to determine rehabilitation needs and potential for achievement.
  • Develop plans of care in accordance with findings.
  • Admit patients and assess therapy needs per OASIS and PPS guidelines.
  • Make initial therapy evaluation visits and re-evaluate patients’ speech-language therapy needs during visits.
  • Conduct follow-up assessments according to OASIS and PPS guidelines.
  • Initiate care plans and ensure safety using patient, family, and community resources.
  • Communicate significant findings and changes to supervisors, physicians, and team members.
  • Participate in implementing and developing Plan of Care for quality care and discharge planning.
  • Verify Plan of Care and provide care per physician orders and guidelines.
  • Write physician orders for additional visits and Plan of Care changes as needed.
  • Incorporate patient care goals into therapy and revise plans as issues arise.
  • Perform direct Speech-Language pathology services per accepted standards.

Job description

  • Promotes Elara Caring’s philosophy, mission statement and administrative policies to ensure quality of care
  • Evaluates the Speech-Language pathologist patient to determine rehabilitation needs and potential for achievement
  • Develops a plan of care in accordance with the findings
  • Admits the patient and assesses speech-language therapy and all other needs according to OASIS and PPS regulatory guidelines
  • Makes the initial therapy evaluation visits and re-evaluates the patient’s speech language therapy needs during each visit
  • Makes follow-up assessments according to OASIS and PPS regulatory guidelines
  • Initiates plan for patient safety, using the patient, family, and community resources
  • Communicates significant findings, problems, and changes in condition or environment to appropriate supervisor, physician, and other team members involved with patient care
  • Participates in implementation and development of Plan of Care to ensure quality and continuity of care and proper discharge planning
  • Verifies Plan of Care prior to each visit and provides care according to physician’s orders, assessment data, and established standards and guidelines
  • Writes physician orders to cover additional visits and changes to Plan of Care, per agency policy
  • Incorporates patient care goals established in Plan of Care, into therapy care
  • Initiates and revises Plan of Care in response to identified patient care issues
  • Performs direct Speech-Language pathology services in accordance with accepted standards of practice
  • Selects diagnostic and therapeutic materials appropriate to patient needs
  • Instructs and provides patients and/or their caregiver in the home therapy program providing modification and clarification when necessary
  • Assures clinical notes indicate continuing communication and coordination of services with physician, other staff, and disciplines
  • Ensures all therapy notes, supply log/visit logs are accurate and turned into office according to office policy
Requirements
  • Master’s degree from an accredited educational program in Language Pathology
  • Current, unrestricted SLP license in the state of work
  • Minimum one (1) year of experience as a SLP in a clinical care setting
  • Able and willing to travel within branch/office coverage area
  • Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws
  • Must be able and willing to travel 50%
  • Able to sit, stand, bend, lift and move intermittently and be able to lift 50-100
