Job summary

PARADIGM REHAB & HEALTH CARE LLC in Keller, Texas is seeking a Speech Language Pathologist PRN. You will perform initial therapy evaluations within 72 hours and develop treatment plans based on evaluations. You will also communicate regularly with health providers and manage your schedule effectively.

The ideal candidate has a Texas Speech Language Pathologist License and at least 1 year of clinical experience in home health. Compensation is based on a per visit rate plus mileage reimbursement.

Qualifications

• Current Texas Speech Language Pathologist License is mandatory.
• Minimum 1 year of clinical experience in home health is required.
• Certification in BSL/CPR must be up-to-date.
• Valid Texas Driver's License and Automobile Insurance.
• Knowledge of WellSky EMR software is a plus.

Responsibilities

Speech Language Pathologist PRN

Responsibilities and Duties

• Perform initial therapy evaluations of each patient within 72 hours of receipt of referral.
• Make opportunities to teach referral sources about determining and recommending appropriate speech and hearing services.
• Communicate regularly with physicians, patients, families, and health care providers to modify plan of care based on patient needs.
• Participate in case coordination regularly with staff nurses and physicians, and document appropriately in medical record.
• Develop treatment plan based on evaluation.
• Provide appropriate clinical follow-up by reporting unfavorable signs and symptoms.
• Manage own schedule with no missed visits or late evaluations due to scheduling errors.
• Instruct other health team personnel and patient.
• Plan for discharge.
