Job summary

CenterWell Home Health is seeking a per diem Speech Language Pathologist to deliver in-home services, evaluate patients, and contribute to care plans. The role emphasizes rehabilitation, independence, and quality of life, with flexible per diem hours based on clinician availability.

You will collaborate with clinical managers, maintain documentation, and ensure compliance with policy while supporting patients and families in home settings.

Qualifications

• Meet the education and experience requirements for ASHA CCC in Speech-Language Pathology or Audiology.
• Minimum six months experience as a speech therapist/speech-language pathologist.
• Current and unrestricted license; CPR certification.
• A valid driver’s license and reliable transportation.

Responsibilities

• Evaluate and provide speech-language pathology services to patients in home or facility.
• Develop and periodically review the Plan of Treatment and Plan of Care.
• Record and report changes in patient condition to supervisor and physician.
• Provide instruction on alternate communication methods when appropriate.
• Document evaluations, visits, care plans, and progress reports per policy.

By delivering personalized care that focuses on rehabilitation and functional improvement, you'll empower individuals to overcome physical limitations, perform everyday activities with confidence and enjoy a better quality of life. Per Diem hours are flexible and will be based on clinician's availability each week. Territory based caseload.

As a per diem Home Health Speech Language Pathologist, you will:

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility Participate in the development and periodic review of the Plan of Treatment and Plan of Care
• Use professional skills and judgment in assessing and treating disorders of speech, voice, language, hearing and swallowing to prevent, identify, evaluate and minimize the effects of such disorders and conditions
• Administer and interpret diagnostic tests and applications of therapeutic treatments including audio logic screening
• Observe, record and report changes in the patient's condition and response to treatment to supervisor and/or the physician
• Provide instruction and training to patients in use of alternative communication systems when appropriate
• Provide counsel and instruction to patients, families and healthcare staff
• Maintain and submit documentation as required by the Company and/or facility
• Prepare and submit written reports of evaluations, visits, summaries, care plans, care coordination activities, and progress reports as required by Company policy
• Participate in care coordination activities and discharge planning
• Maintain the highest standards of professional conduct concerning information that is confidential in nature
• Share information only when the recipient's right to access is clearly established and the sharing of such information is clearly in the best interests of the patient

Required Qualifications:

• Meet the education and experience requirements for Certification of Clinical Competence in Speech Language Pathology or Audiology granted by ASHA
• Minimum six months experience as a speech therapist / speech language pathologist
• Current and unrestricted license
• Current CPR certification
• A valid driver's license, auto insurance, and reliable transportation are required.

Pay Per Visit/Unit Rate

$54.00 - $76.00

Scheduled Weekly Hours 1 Pay Range The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

TB Statement:

This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Driving Statement:

This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance. Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher.
