Job summary

Park Highlands Nursing is seeking a Per Diem Speech Language Pathologist (SLP) to support our in-house team. You will evaluate residents for swallowing and cognitive deficits, develop individualized plans, and provide AAC interventions to improve independence.

Education for families and collaboration with care staff are essential components of the role. Responsibilities include maintaining accurate documentation and coordinating with physicians for swallow studies as ordered, while ensuring

Qualifications

• Experience in SNF or long-term care settings preferred.
• Current knowledge of speech-language pathology treatment practices.

Responsibilities

• Evaluate swallowing or cognitive deficits in residents.
• Help residents improve cognition or swallowing to enhance independence.
• Provide skill-based speech-language therapy assessments and develop individualized treatment plans.
• Administer AAC interventions for individuals with expressive/language challenges.
• Educate residents, caregivers, and other professionals.
• Coordinate with physicians for diagnostic swallowing procedures as ordered.
• Document billing records and clinical notes accurately.
• Maintain confidentiality and comply with regulations.

Job description

We have an amazing opportunity for aPer Diem (PRN) Speech Language Pathologist!

We are looking for aSLPto join our in-house team of outstanding professionals that service our residents through person-centered care to achieve positive outcomes!

SLP Responsibilities:

• Evaluate Residents conditions for swallowing or cognitive deficits.


• Help Residents in their goals to improve Cognition or Swallowing


• Education for Residents and Caregivers


• Provide skilled, medically necessary speech and language therapy assessments as well as develop and implement individualized treatment plans to increase client’s independence and functioning.


• Provide augmentative and alternative communication (AAC) systems for individuals with severe expressive and/or language comprehension disorders.


• Provide training and education to clients, family, caregivers, and other professionals as a subject matter expert.


• Coordinate and collaborate with other professionals for diagnostic swallowing procedures (MBSS, FEES) as ordered by physician.


• Demonstrate competent technical skills according to current Speech Therapy standards of practice.


• Communicate changes in resident’s condition.


• Provide accurate and timely billing records and supporting clinical documentation.


• Respond to family requests and concerns timely.


• Coordinate and provide training with Restorative Nursing and other Care staff.


• Assure resident abuse or neglect does not occur.


• Observe confidentiality and safeguards for all patient information.


• Complies with federal, state, and local legal and professional requirements.

SLP Skills:

• SNF/Long term care experience preferred.


• Current knowledge of treatment practices


• Knowledge of Net Health Electronic Documentation System preferred.


• Ability to manage patients with different types of personalities.

SLP Requirements:

• Active/Valid Texas license in Speech-Language Pathology


• Current ASHA Certification


• In good standing with all regulatory agencies and licensing boards
