Job summary

JTC Home Health is seeking a Speech-Language Pathologist (SLP) to work on a per diem/contract basis in San Diego, California. The role involves conducting evaluations and developing treatment plans for patients with speech and swallowing disorders in their home environment.

The ideal candidate will hold a Master's degree in Speech-Language Pathology, have a valid California SLP license, and at least one year of relevant experience. You will be part of a supportive clinical and administrative team, with compensation based on patient visits and mileage reimbursement.

Qualifications

• Minimum of one (1) year of experience as an SLP, with at least six (6) months in a certified Home Health agency preferred.
• Must have a reliable car, valid driver's license, and current auto insurance.
• Ability to travel throughout San Diego and surrounding counties.

Responsibilities

• Conduct comprehensive initial and ongoing speech-language pathology evaluations in the patient's home.
• Develop, implement, and document individualized treatment plans for patients with speech, language, cognitive, and swallowing disorders.
• Provide patient/caregiver education and training on home programs and compensatory strategies.
• Coordinate with physicians and other members of the healthcare team.
• Complete timely and accurate OASIS and documentation requirements.

Compensation & Benefits

• Routine Visit: $110.00 per visit

Responsibilities

• Conduct comprehensive initial and ongoing speech-language pathology evaluations and assessments in the patient’s home environment.
• Develop, implement, and document individualized treatment plans (POC) for patients with speech, language, cognitive, and swallowing disorders (dysphagia).
• Provide patient/caregiver education and training on home programs and compensatory strategies.
• Coordinate with physicians, nurses, and other members of the interdisciplinary healthcare team.
• Complete timely and accurate OASIS and documentation requirements using an Electronic Medical Record (EMR) system.
• Maintain open and professional communication with the JTC Home Health office team.

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program.
• Current and unencumbered California SLP license.
• Certificate of Clinical Competence (CCC-SLP) from ASHA required.
• Minimum of one (1) year of experience as an SLP, with at least six (6) months in a certified Home Health agency preferred.
• Knowledge of Medicare/Medicaid regulations and documentation requirements (OASIS experience preferred).
• Must have a reliable car, valid driver's license, and current auto insurance.
