Qualifications

• Must have a valid SLP license in the state of practice.
• Graduate of ASHA-accredited program; foreign-trained must be deemed equivalent by a credentialing organization.

Responsibilities

• Complete evaluations, assessments, and treatments per care plan.
• Complete all documentation in a timely, accurate manner.
• Follow established policies and procedures related to scope of practice.
• Recognize and respond to priorities in patient care with initiative and sound judgment.
• Complete required orientations, in-services, and trainings on time.
• Perform other related duties as directed by supervisor.
