Home Health Speech Therapist – King County

Signature Healthcare at Home is a leading provider of home health in Oregon and Washington. The mission is to keep patients in their homes. The role is field‑based serving clients across Federal Way and surrounding cities.

Compensation: $50‑$60 per hour. Type: PRN, Part‑time or Full‑time. Work schedule: Monday – Friday, 8:00 AM – 5:00 PM.

Job Summary

Join a supportive and collaborative therapy team where you will work closely with an experienced home health SLP for mentorship, ongoing clinical support, and be part of a culture that encourages professional growth.

Responsibilities
  • Conduct appropriate evaluations, assess home environment (as appropriate), and identify equipment needs relative to speech/language function.
  • Assume responsibility for the interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, representative (if any), and caregiver(s).
  • Provide services ordered by the physician as indicated in the plan of care.
  • Provide patient, caregiver, and family counseling and education.
  • Prepare clinical notes.
  • Communicate with the physician responsible for the home health plan of care and other health care practitioners (as appropriate) related to the current home health plan of care.
  • Identify functional speech defects and establish a plan of care to improve patient’s function.
  • Administer speech therapy program utilizing specialized therapeutic technique and/or equipment.
  • Recommend communication devices/aids as indicated.
  • Participate in case conferences, in‑services, and other meetings required ensuring coordinated and comprehensive care.
  • Maintain clinical competency in speech language pathology practice and theory.
  • Identify patient and family/caregiver needs for other services and refer as appropriate.
  • May supervise home health aide in procedures directly related to the speech‑language therapy plan of care.
  • Participate in the HHA's quality assessment and performance improvement program and HHA-sponsored in‑service training.
  • Perform other duties as required to facilitate the delivery of speech language pathology services.
Requirements
  • Must have a master’s or doctoral degree in speech‑language pathology and be licensed in the state where practicing (or have completed or be in the process of completing 350 supervised clinical hours).
  • In such cases, the speech‑language pathologist must have performed not less than nine months of supervised full‑time services after obtaining a master’s or doctoral degree and successfully completed a national examination in speech‑language pathology approved by the Secretary.
  • A minimum of one year’s clinical experience in Speech‑Language Pathology.
  • Demonstrates good verbal and written communication and organization skills.
  • Possesses and maintains current CPR certification.
  • Ability to drive and/or have access to transportation; must have a valid driver’s license, maintain automobile insurance coverage, and have access to an automobile.
