Job summary

FOX Rehabilitation is expanding in Fairfax, VA and seeks licensed Speech Language Pathologists to deliver one-on-one care to older adults in homes or Senior Living Communities. You’ll develop individualized plans, provide evidence-based therapy, and document in FOX’s EMR while collaborating with an interdisciplinary team.

Qualifications

• Active license in the state or eligibility to obtain
• Master's degree in Speech-Language Pathology
• Current ASHA CCC credential
• Current CPR certification
• Proficiency with EMR systems

Responsibilities

• Provide one-on-one speech language pathology services to older adults in homes or senior living communities
• Perform evaluations and develop individualized plans of care
• Deliver evidence-based therapy sessions
• Document in FOX’s EMR per regulatory standards
• Collaborate with PTs, OTs, SLPs and Fitness Specialists
• Educate patients, caregivers and staff to promote safety and aging in place
• Offer telehealth/virtual care when appropriate

Responsibilities

• Provide one-on-oneSpeech Language Pathologyto older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
• Document effectively within FOX’s EMR systemin accordance withregulatory standards
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
• Provide telehealth / virtual care whenappropriate toextend access and flexibility

Qualifications

• ValidSpeech Language Pathologylicense in the state(s) of practice, or eligibility to obtain
• Degree from an accreditedSpeech Language Pathologyprogram

• Current Certificate of Clinical Competence (ASHA CCC)

Physical Requirements

FOX clinicians must be able to meet the physical demands of in-home and community-based care, including:

• Exerting medium physical effort, including lifting 25–50lbsoccasionally, 10–25lbsfrequently, and up to 10lbsconstantly
• Performing CPR
• Providing moderate to maximumassistancewith patient transfers and bed mobility
• Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities
• Driving to patient homes and senior living communities as part of the Geriatric House Calls™ model
• Wearing PPE whenrequired
