Therapy Management Corporation (TMC) is seeking a Speech-Language Pathologist for CFY with mentorship and a new graduate bonus to support adult/geriatric patients. You will provide speech therapy services, develop client-centered plans, and collaborate with interdisciplinary teams to enhance communication and quality of life.

Qualifications

• Licensed Speech Language Pathologist (SLP) or license eligible in the state of practice.
• Graduate of an accredited Speech Language Pathologist program.
• Interest or experience in geriatric rehabilitation or senior living therapy.
• Passion for patient-centered care and improving quality of life for older adults.

Responsibilities

• Evaluate patients and provide personalized speech therapy services that improve communication, cognition, swallowing function and overall quality of life.
• Develop and implement patient-centered treatment plans focused on speech, language, cognition and swallowing rehabilitation goals.
• Deliver evidence-based therapy interventions that support functional independence in adult and geriatric patients.
• Partner with interdisciplinary teams, caregivers, and nursing staff to support strong patient outcomes and continuity of care.
• Maintain accurate clinical documentation, monitor patient progress and adjust treatment plans based on individual rehabilitation needs.
