Job summary

FOX Rehabilitation is seeking a licensed Speech Language Pathologist to provide one-on-one care for older adults in private residences and senior living communities. You will deliver evaluations, create individualized plans, and administer evidence-based therapy, with the autonomy to manage your own schedule.

The role supports telehealth options and collaboration with an interdisciplinary team to promote safety, function, and aging in place.

Qualifications

• Must hold a valid Speech Language Pathology license in the practicing state or be eligible to obtain.
• Degree from an accredited Speech Language Pathology program.
• Basic computer literacy and EMR proficiency.
• Current CPR certification.
• Current Certificate of Clinical Competence (ASHA CCC) as applicable.

Responsibilities

• Provide one-on-one Speech Language Pathology services to older adults in home or community settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support wellness, mobility, and functional independence.
• Document effectively in FOX’s EMR system in compliance with standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.

Physical Requirements

FOX clinicians must be able to meet the physical demands of in-home and community-based care, including:

• Exerting medium physical effort, including lifting 25–50lbsoccasionally, 10–25lbsfrequently, and up to 10lbsconstantly
• Performing CPR
• Providing moderate to maximumassistancewith patient transfers and bed mobility
• Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities
• Driving to patient homes and senior living communities as part of the Geriatric House Calls model
• Wearing PPE whenrequired
