Job summary

Springfield Health Care Center in Ohio seeks a Speech Language Pathologist to plan and direct speech language pathology and audiology services in accordance with physician orders and ASHA standards. You will evaluate and treat residents, document and code services, participate in interdisciplinary teams, and support resident rights and infection control while promoting teamwork and quality outcomes.

Job description

We are currently seeking a Speech Language Pathologist.

Job Responsibilities and Duties
  • Plan, develop, implement, and direct speech language pathology and audiology services in accordance with physician orders, state and federal regulations, and ASHA standards.

  • Provide comprehensive evaluations, treatment plans, therapeutic interventions, and discharge planning for residents receiving speech language pathology services.

  • Ensure timely, accurate, and compliant documentation, billing, and coding for all speech language pathology and audiology services.

  • Provide required resident assessment data, including swallowing function, speech and hearing patterns, cognitive patterns, BIMS scores, therapy minutes, and observation period documentation.

  • Participate as a qualified clinician in interdisciplinary functional and cognitive assessments for Medicare Part A and Medicare Advantage residents upon admission and as required.

  • Coordinate and participate in interdisciplinary team meetings to discuss therapy progress and discharge planning.

  • Monitor and report speech language pathology and audiology key performance indicators to the Director of Rehabilitation and recommend corrective actions when necessary.

  • Ensure speech language pathology and audiology equipment is properly stored, used, cleaned, sanitized, and maintained safely.

  • Maintain compliance with state Speech Language Pathology and Audiology Practice Acts and professional association guidelines.

  • Collaborate with physicians, nursing staff, consultants, and community agencies to enhance resident outcomes.

  • Build and maintain positive relationships with residents, families, and staff through clear, respectful communication.

  • Promote teamwork, accountability, and effective problem-solving within the rehabilitation department.

  • Participate in facility committees, audits, surveys, and regulatory inspections as required.

  • Promote safe work practices, infection control standards, and resident rights at all times.

Requirements
  • Valid, unrestricted Speech Language Pathologist license in the state of practice.

  • Meets all educational requirements for speech language pathology licensure.

  • Valid driver’s license in the state of employment.

  • Knowledge of skilled nursing facility regulations, long-term care laws, and rehabilitation compliance standards.

  • Strong clinical judgment, documentation, and communication skills.

  • Ability to work collaboratively with interdisciplinary teams.

  • Commitment to confidentiality, HIPAA compliance, and resident rights.

  • Physical ability to move throughout the facility and assist during emergency situations.

  • Willingness to work beyond normal hours, including weekends and holidays when necessary.
