AdventHealth Shawnee Mission in Kansas seeks a Speech-Language Pathologist for per diem (PRN) Home Health, working day shifts. The role involves documenting treatments, updating care plans, and writing modified orders in the patient chart, with supervision duties for aides and participation in care meetings.

The ideal candidate holds a Master’s in Speech-Language Pathology, valid SLP license, and a driver’s license, with at least 1 year of clinical SLP experience in Medicare-certified home

Formación

• Master's degree in Speech-Language Pathology is required.
• 1+ year of clinical speech therapy experience is required.
• Recent Medicare-certified home health agency case-management experience is preferred.
• SLP licensure and Driver's License are required.

Job Description

• 24-28 hours a week, No weekends or on call
• Documents treatment provided in medical records, updating care plans and writing modified orders for changes in the plan of care.
• Completes all documentation in a timely and accurate manner.
• Participates in clinical record reviews, agency staff meetings, therapy meetings, and other activities as requested.
• Provides supervision and guidance to aides, ensuring compliance with care plans and documentation requirements.
• Performs evaluations and utilizes findings to determine diagnosis (for SLP services), prognosis, and plan of care in consultation with the physician.
• Determines specific interventions, duration, and frequency needed to achieve anticipated goals and outcomes.
• Observes and records activities and findings in the clinical record, reporting patient reactions and changes to the physician or primary Case Manager.
• Assess patient responses to interventions and performs reassessments as required.
• Instructs other health team personnel in phases of speech therapy relevant to patient care.
• Instructs patients and caregivers on speech language improvement programs, including adaptive equipment and home programs.
• Selects, applies, or modifies skilled interventions to achieve the best patient outcomes.
• Performs other duties as assigned.

• A working knowledge of community resources and an ability to refer patients and families appropriately [Preferred]
• Home Care Regulations and Third‑Party Reimbursement as it impacts care delivery [Preferred]

Work Experience

• 1+ relevant clinical speech therapy experience [Required]
• Recent, relevant experience in a Medicare‑certified home health agency as a case‑manager [Preferred]
