Job summary

Creekside Transitional Care & Rehab in Meridian, ID is seeking a full-time Speech-Language Pathologist for our in-house rehab program. You will evaluate patients with communication, cognitive, or swallowing disorders and develop tailored care plans to restore function.

You will deliver rehabilitative treatment, monitor progress, provide counseling to patients and families, and generate comprehensive discharge summaries in line with regulatory and clinical standards.

Qualifications

• Current Speech-Language Pathologist license is required.
• Open to SLPs at all experience levels.
• Ability to work in a post-acute rehab setting with geriatric patients.

SPEECH-LANGUAGE PATHOLOGIST - FULL TIME - CREEKSIDE TRANSITIONAL CARE & REHABILITATION - MERIDIAN, IDAHO

Creekside Transitional Care & Rehab in Meridian, ID is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a full time Speech-Language Pathologist for our in-house rehab program!

Key Responsibilities:

• Screen and evaluate patients with communication, cognitive, or swallowing disorders, developing appropriate care plans following regulatory and clinical standards.
• Deliver rehabilitative treatment to patients with communication, cognitive, and/or swallowing disorders, ensuring compliance with regulatory and clinical practice requirements.
• Evaluate treatment outcomes, modifying services based on patient evaluation and making referrals as needed.
• Provide consultation and counseling to patients, families, caregivers, and other service providers related to speech disorders.
• Generate comprehensive discharge summaries in adherence to regulatory and clinical requirements.

Qualifications:

Speech-Language Pathologist license is required.
