Job summary

Phoenix Home Care and Hospice is seeking a speech-language pathologist to provide direct therapy to home health patients, diagnosing and treating speech, language, cognitive, and swallowing disorders in the patient’s home. Requirements include a Master’s degree in Speech-Language Pathology and current state licensure; 1 year of clinical experience preferred.

Must be able to travel, communicate effectively, and maintain thorough documentation.

Qualifications

• Master's degree in Speech-language pathology and current state license.
• Prefer one year of clinical experience, preferably in acute care or rehabilitation.
• Ability to work in a fast-paced environment with good judgment, flexibility and patience.
• Mental alertness, precision, analytical ability, problem solving and initiative.
• May require frequent travel and irregular hours.

Responsibilities

• Performs direct speech therapy services to patients as ordered by a physician and in accordance with the Plan of Care.
• Complies with agency standards, policies, procedures, state and federal regulations and payer requirements.
• Provides legible, accurate, timely documentation in accordance with agency standards.
• Communicates and collaborates with team members when reviewing and updating the Plan of Care.
• Performs duties in alignment with agency financial and productivity goals.
• Performs any other duties as assigned by supervisor.

Responsibilities

• Performs direct speech therapy services to patients as ordered by a physician and in accordance with the Plan of Care
• Complies with agency standards, policies, procedures, state and federal regulations, state practice acts for licensed personnel, the Business Code of Ethics, and payer requirements
• Provides legible, accurate, timely and complete documentation in accordance with the established agency standards of all care and services provided to patients as assigned
• Communicates and collaborates with team members when reviewing, revising, and updating the patient\'s Plan of Care
• Performs job responsibilities in accordance with agency financial and productivity goals in the promotion of effective and efficient agency operations
• Performs any other duties as assigned by supervisor

Requirements

• Education / Licensure: Master\'s degree in Speech-language pathology. Current license in the state(s) of practice
• Experience: Prefer one (1) year clinical experience, preferably in an acute care or rehabilitation setting
• Ability to work in a fast-paced, stressful environment, utilizing good judgment, flexibility and patience in all situations
• Position requires mental alertness, precision, analytical ability, problem solving, memory, communication, concentration and initiative
• May require frequent travel and irregular hours
