Job summary

AdventHealth Shawnee Mission is seeking a licensed Speech Language Pathologist to join our Medicare-certified home health team. This part-time role entails documenting treatment, updating care plans, performing evaluations, and coordinating with the physician and Case Manager to optimize patient outcomes.

You will supervise aides, educate patients and families on speech language strategies, and ensure timely, accurate charting. No weekends or on-call duty, with competitive benefits from Day One.

Qualifications

• Master's degree in Speech Language Pathology is required.
• 1+ year of relevant clinical speech therapy experience.
• Recent experience in a Medicare-certified home health agency is preferred.

Responsibilities

• Document treatment in medical records and update care plans.
• Provide supervision to aides and ensure care plan compliance.
• Perform evaluations to determine diagnosis, prognosis, and plan of care in consultation with physician.
• Instruct patients and caregivers on speech language programs and home programs.
• Collaborate with physician and Case Manager on interventions and outcomes.
• Complete documentation timely and accurately.

Job Description:

• 24-28 hours a week, No weekends or on call
• Documents treatment provided in medical records, updating care plans and writing modified orders for changes in the plan of care.
• Completes all documentation in a timely and accurate manner.
• Participates in clinical record reviews, agency staff meetings, therapy meetings, and other activities as requested.
• Provides supervision and guidance to aides, ensuring compliance with care plans and documentation requirements.
• Performs evaluations and utilizes findings to determine diagnosis (for SLP services), prognosis, and plan of care in consultation with the physician.
• Determines specific interventions, duration, and frequency needed to achieve anticipated goals and outcomes.
• Observes and records activities and findings in the clinical record, reporting patient reactions and changes to the physician or primary Case Manager.
• Assesses patient responses to interventions and performs reassessments as required.
• Instructs other health team personnel in phases of speech therapy relevant to patient care.
• Instructs patients and caregivers on speech language improvement programs, including adaptive equipment and home programs.
• Selects, applies, or modifies skilled interventions to achieve the best patient outcomes.
• Performs other duties as assigned.

Knowledge, Skills, and Abilities:

• Functions with a high degree of independence [Required]

• A working knowledge of community resources and an ability to refer patients and families appropriately [Preferred]

Work Experience:

• 1+ relevant clinical speech therapy experience [Required]
• Recent, relevant experience in a Medicare-certified home health agency as a case-manager [Preferred]
