Job summary

Optimal Home Care is seeking a Speech-Language Pathologist to provide in-home services for patients in the Colorado Springs area. The clinician evaluates speech and language abilities, plans rehabilitation, and collaborates on the Physician’s Plan of Care.

Responsibilities include documenting progress, maintaining patient records, and ensuring HIPAA compliance while delivering high-quality care with a patient-centered approach.

Qualifications

  • Master's or Doctoral Degree in Speech-language Pathology or Audiology.
  • Licensed as a Speech-Language Pathologist in Colorado in good standing.
  • Strong written and verbal communication skills.
  • Excellent interpersonal skills.

Responsibilities

  • Evaluates patient speech and language abilities and rehabilitates speech, language, and dysphagia disorders.
  • Plans and provides rehabilitative services and contributes to the patient's Plan of Care.
  • Documents progress notes weekly and maintains patient records.
  • Communicates with patients and family to support outcomes and discharge planning.
  • Adheres to HIPAA, policy procedures, and professional standards.

Education

Position & Responsibilities:
  • Complies with ethical and professional standards of conduct as set forth by the American Speech Hearing and Language Association.
  • Practices and maintain patient safety and dignity by abiding by Federal, State, Local and Optimal policy & procedures as pertains to patient protected health information and HIPAA (Health Information Portability and Accountability Act).
  • Evaluates patient speech and language abilities, both defects and assets.
  • Plans and provides rehabilitative services for speech, language and dysphagia disorders.
  • Participates in the development of the Physician’s Plan of Care.
  • Develops, documents, implements and re-evaluates the Patient’s Plan of Care.
  • Records clinical/progress notes to the patient’s clinical record weekly.
  • Records type of treatment and patient’s reaction to it. Reports progress and/or regression.
  • Maintains adequate records on all patients.
  • Utilizes community resources and agency personnel by proper referral.
  • Selects and administers diagnostic and therapeutic techniques and materials.
  • Instructs and counsels other health team personnel and family members in methods of assisting patient in improving, correcting and accepting his/her disabilities.
  • Plans for patient’s discharge, prepares discharge summaries and instructions.
  • Attends agency service meetings and participates in staff development activities.
  • Performs related and other duties as required.
  • Complies with agency Policies and Procedures as well as Employee Handbook detailing Human Resources policies.
  • Other duties / responsibilities that support the agency’s core values of Commitment, Advocacy, Respect, and Excellence.
  • Completes emergency preparedness training as identified as appropriate for this employee level.
  • Participates in exercises and drills for emergency preparedness, as required.
  • Other duties and tasks as assigned by the Optimal Home Care Emergency Command
  • Center communication tree in the event of a local, regional or national emergency and/or disaster.

Requirements
Qualifications:
  • Has a Master’s or Doctoral Degree in Speech-language Pathology or Audiology and who has preferably one (1) full year of supervised work experience.
  • Must be currently licensed as a speech-language pathologist in good standing in the state of Colorado.
  • Must be able to communicate in the language of the client and document client related activity as appropriate.
  • Possesses adequate and effective written and verbal communication skills.
  • Possess adequate and effective interpersonal skills.
