Job summary

UnityPoint Health’s Methodist Hospital Specialty Clinic is seeking a compassionate Registered Nurse (RN) to join the Endocrinology team. You will triage patient needs, coordinate care, educate patients and families, and support management of chronic endocrine conditions, including diabetes and thyroid disorders.

Qualifications

• RN license active/unencumbered in state of practice or compact license.
• ADN required; BSN preferred.
• For UnityPoint Clinic positions, may float to other locations to support staffing.

Responsibilities

• Evaluate incoming patient calls to determine urgency and next steps.
• Provide telephone triage and guide patients to appropriate care or follow-up.
• Assess concerns related to diabetes, thyroid disorders, hormonal imbalances, and other endocrine conditions.
• Coordinate care for patients with complex chronic endocrine conditions.
• Communicate with endocrinologists and clinical team to ensure timely care.
• Document patient interactions and care plans in the electronic health record (EHR).
• Follow up on test results, medication changes, and provider recommendations.
• Support patient education on endocrine conditions, treatment plans, and lifestyle modifications.

Responsibilities

• Evaluate incoming patient phone calls to determine the urgency of symptoms and appropriate next steps.
• Provide telephone triage and guide patients to the appropriate level of care or follow-up.
• Assess patient concerns related to diabetes, thyroid disorders, hormonal imbalances, medication changes, and other endocrine conditions.
• Assist in coordinating care for patients with complex or chronic endocrine conditions, including diabetes, thyroid disorders, adrenal disorders, osteoporosis, and other hormonal conditions.
• Communicate efficiently with endocrinologists and clinical team members to ensure timely and effective care.
• Coordinate follow-up appointments, testing, referrals, and other aspects of the patient’s plan of care.
• Educate patients and caregivers on endocrine conditions, treatment plans, medications, lifestyle modifications, and strategies for managing chronic conditions.
• Reinforce diabetes self-management, including blood glucose monitoring, medication adherence, and recognizing signs and symptoms that require additional medical attention.
• Accurately document patient interactions, assessments, education, and care plans using the electronic health record (EHR).
• Follow up with patients regarding test results, medication changes, treatment plans, and provider recommendations.
• Track patient outcomes and coordinate follow-up care to promote continuity and positive health outcomes.

Qualifications

• Registered Nurse (RN) – Active/Unencumbered RN license in state of practice or compact license including state of practice.
• All levels of RN experience will be considered. Prior experience in endocrinology, diabetes care, or specialty care is a plus, but not required.
• Associate Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred.
• For positions within UnityPoint Clinic, team members may be required to work at or to float to other locations to support optimized staffing.
