Qualifications

• Requires Virginia Pharmacist license and pharmacy degree.
• BLS/ACLS certifications preferred within role.
• Hospital experience and PGY1/PGY2 residency training preferred.

Responsibilities

• Manages patient drug therapy according to clinical practice standards.
• Ensures appropriateness of provider medication orders for patient safety.
• Provides in-depth patient counseling and education when required.
• Precepts pharmacy students and residents; mentors staff as needed.
• Leads system and local initiatives; supports policy and training material development.
