Job summary

Clearway Health in Bowling Green, KY is seeking a Clinical Pharmacist to optimize patient outcomes in chronic and specialty therapy, with a $10,000 sign-on bonus. You will prevent or resolve medication-related issues, educate patients and healthcare teams, and coordinate drug therapy as needed.

You will collaborate with prescribers and payors, support prior authorizations, monitor outcomes, and ensure compliance with URAC/ACHC standards.

Qualifications

• Pharmacist degree and relevant residency training.
• Clinical pharm experience in direct patient care preferred.
• Licensure in good standing to practice in the applicable state.

Responsibilities

• Perform comprehensive medication therapy reviews for patients on specialty meds.
• Provide disease-specific counseling to enhance understanding and adherence.
• Collaborate with prescribers, nurses, case managers, and payors for therapy initiation/continuation.
• Assist with prior authorization, appeals, and benefits verification.
• Monitor outcomes and adherence metrics to assess effectiveness.
• Document interventions and outcomes for quality improvement and audits.
• Ensure adherence to URAC/ACHC standards and payor requirements.
• Maintain documentation to support audits and accreditation readiness.
• Contribute to developing clinical protocols and formulary strategies.
• Support initiatives to improve workflow and patient satisfaction.
