A Brief Overview

Reviews and processes medication orders using the hospital information system (HIS) and the pharmacy computer system to ensure appropriateness regarding dose, dosage form, route of administration, frequency, duration of therapy, and patient's disease state. Supervises pharmacy technicians and interns to ensure pharmacy products are prepared and distributed accurately, with the appropriate quantity and at the needed time. When necessary, the pharmacist prepares and distributes pharmacy products accurately, in the appropriate quantity, and at the needed time.

What you will do

  • Review and process medication orders using the hospital information system (HIS) and pharmacy computer system to ensure appropriateness regarding dose, dosage form, route of administration, frequency, duration of therapy, and patient's disease state.
  • Ensure medication orders are appropriate for age, weight, and disease state 100% of the time.
  • Review medication orders for potential allergic reactions, drug interactions, therapeutic duplication, and incompatibilities 100% of the time.
  • Ensure PRN medication orders contain indications and are documented 100% of the time.
  • For medication orders outside the normal range, ensure the reason and rationale for use are appropriate and documented in the pharmacy intervention record.
  • Provide drug information and/or educational programs to healthcare professionals and patients.
  • Consult with physicians, nurses, and other healthcare professionals concerning drug dose, dosage form, availability, proper administration, side effects, and incompatibilities.
  • Advise patients and families on medication use, including name, description, route, dosage, duration, administration directions, side effects, interactions, contraindications, self-monitoring, storage, refill info, and actions for missed doses.
  • Provide training to new pharmacy staff as assigned.
  • Ensure compliance with CHOP Formulary System.
  • Review non-formulary medication orders for appropriateness or obtain approval from a clinical pharmacist.
  • Approve non-formulary medication orders when authorized, ensuring use is appropriate and documented.
  • Ensure adequate inventory for therapy completion.
  • Supervise pharmacy technicians and interns to ensure accurate preparation and labeling of unit dose medications and compounded sterile preparations in accordance with policies and standards.
  • Prepare sterile and non-sterile compounded medications following USP standards and department procedures.
  • Maintain proper medication storage and compatibility, ensuring stability within anticipated use time.
  • Replace floor stock medications as per policy.
  • Properly repackage doses and document quality control processes.
  • Verify the content of patient medication cassettes for accuracy.
  • Retrieve discontinued or expired medications properly and communicate changes effectively.
  • Ensure medication availability during dosage or frequency changes.
  • Maintain medication turnaround times: ≤1 hour for regular orders, ≤15 minutes for stat orders.
  • Report and document medication errors immediately and appropriately.
  • Ensure proper documentation and secure storage of controlled substances.
  • Participate in CQI activities and Lean projects to improve departmental processes.
  • Utilize Lean principles for problem-solving and process improvement, including 5S, waste elimination, JIT, and standard work.
  • Identify inefficiencies and recommend solutions.

Education Qualifications

  • Bachelor's Degree in Pharmacy required
  • Doctorate in Pharmacy preferred

Skills and Abilities

  • Proficiency with pharmacy information systems, automated dispensing, and computers (required)
  • Effective interpersonal and communication skills (required)
  • Knowledge to provide care to neonates, children, and adolescents (preferred)

Licenses and Certifications

  • Licensed Pharmacist in Pennsylvania (required upon hire)
